Travel Screening Flier V3.0
Serious Communicable Disease Travel Screening questions must be asked for ALL patients upon
arrival at a check in desk and electronically documented during the rooming process in
powerchart. Emory Healthcare follows the CDC’s guidelines for travel screening.
Effective March 23, 2016:
Please ask all patients the following questions at this point in the check-in process:
1) Have you traveled outside the United States or Canada in the past 21 days?
If the patient responds with “Yes”, go to question 2
2) Where have you traveled outside the United States or Canada?
If the patient responds with any of the following countries or areas, go to question 3.
Middle East/Arabian Peninsula
Countries or areas considered in the Middle East/Arabian Peninsula include: Bahrain, Iraq, Iran,
Israel, West Bank, Gaza, Jordan, Kuwait, Lebanon, Oman, Qatar, Saudi Arabia, Syria, United
Arab Emirates, and Yemen
3) Do you have any of the following symptoms? (Read list of symptoms)
Fever, Chills/Rigor, Weakness, Muscle Pain, Headache
Cough, Shortness of Breath, Sore Throat
If a fever or cough is present, ask the patient, their family or accompanying parties to wear a
surgical/procedural mask from your Infection Prevention Station.**
If yes to questions above, notify clinical staff immediately by stating “we have a Code Buddy” in a
private manner to not cause commotion.
Scripting to patient: “Please wait here, I’ll have a clinical team member escort you to a room.”
Scripting if patient has questions regarding screening: “Emory Healthcare is following CDC
travel screening guidelines to ensure the safety of our patients.”
**If you do not have an Infection Prevention Station in your arrival area, please contact purchasing
to request a quote. The station that we use is item # ICF-2TX. Clinics are responsible for ensuring
that these stations are available to patients in the lobby/check in area/waiting rooms.

EHC Entrance Screening Summary
To ensure the safety of our patients, visitors and staff Emory Healthcare is screening all patients, visitors, and vendors
who enter our hospitals, ambulatory and administrative locations. As a reminder, Emory Healthcare expects all staff to
self- screen before coming to work and staff should not arrive if ill. Emory Healthcare facilities will display Entrance
Screening signage at all points of entry and screening stations.
Screening Process
1. Have you experienced any of the below symptoms in the last 2 weeks, or have you had a COVID test within
the last 14 days due to COVID-related symptoms?
• Fever (temperature > 100 F)
• Sore throat
• Shaking/chills
• Body/muscle aches
• Headache
• Loss of taste or smell
• Diarrhea
• Cough
• Shortness of breath or difficulty breathing
• Rash
2. Proceed to take individual’s temperature.
3. Based on the symptom and temperature screening, screener will follow the steps outlined below:
Asymptomatic Screen
Individual not experiencing any symptoms above AND temperature is below 100 F AND
has not had a COVID test within the last 14 days due to COVID-related symptoms.

today’s date in BLACK marker. The sticker
should be placed on the chest where it is easily
