In this video, we demonstrate the technique of intradermal medication administration.
<p>In the 2022 monkeypox outbreak, this technique is being used to administer the Jynneos smallpox and monkeypox vaccine in order to increase the number of available JYNNEOS vaccine doses. <br /><br />The standard, FDA-approved regimen involves a subcutaneous injection of 0.5mL of the Jynneos vaccine. The standard regimen continues to be authorized for people under 18 years of age, under an Emergency Use Authorization. <br /><br />However under an Emergency Use Authorization beginning August 9, 2022, an alternative regimen may be used for people 18 years of age or older. The authorized alternative regimen involves an intradermal route of administration with an injection volume of 0.1mL. <br /><br />The Jynneos vial in the video contains 0.5ml as a single-dose subcutaneous injection. In this video, we demonstrate how to use one single 0.5ml vial to draw up 5 intradermal doses of 0.1ml. <br /><br />Important medication information can be found on the vial, including the lot number and manufacturer. The expiration date can be found on the side of the box. Regardless of this expiration date, each vial, once accessed is only good for 8 hours. <br /><br />To perform the intradermal injection of the vaccine, follow these steps:</p>
<ol>
<li>Locate the injection site on the forearm, 2-3 inches below the antecubital fossa.</li>
<li>Clean injection site, and allow to air dry.</li>
<li>Hold the skin taut with your hand, either from below or by holding the skin distal to the proposed injection site.</li>
<li>With the bevel of the needle UP, insert the needle at a 5-15 degree angle under the skin until the entire bevel is within the skin, the entire needle does not need to be inserted.</li>
<li>Flatten the angle to the skin.</li>
<li>Inject the vaccine slowly to create a wheal or blister.</li>
<li>Withdraw the needle at the same low angle.</li>
<li>Do not apply pressure following administration; instruct your patient not to apply pressure to avoid displacement of vaccine into deeper skin tissue. Troubleshooting and injection issues: If no wheal is created, a portion of the vaccine solution leaks back out the injection site, or if bleeding occurs and no wheal or bleb remains, repeat the dose at another site, at least 2 inches away from the first injection site.</li>

<h3>This is a collection of Contingency and Crisis capacity resources. What are Contingency and Crisis capacities?</h3>
<p style="margin-left:1.25rem;">This collection of resources contains recommendations facilitating conservation of equipment and supplies during <strong>contingency</strong> (expected shortages) and <strong>crisis</strong> (known shortages) capacities and <em><strong>should not be applied</strong></em> as guidance when <strong>conventional capacities</strong> are available.</p>
<p style="margin-left:1.25rem;">Contingency and then crisis capacity measures augment conventional capacity measures and are meant to be considered and <strong>implemented sequentially</strong> (<a href="https://www.cdc.gov/niosh/topics/pandemic/strategies-masks.html" target="_blank" rel="noreferrer noopener">CDC</a>). They are recommended in the following sequence:</p>

<ul style="margin-left:2rem;">
<li><strong>Conventional capacity</strong> include measures consisting of engineering, administrative, and personal protective equipment (PPE) controls that should already be implemented in general infection prevention and control plans in healthcare settings.</li>
<li><strong>Contingency capacity</strong> measures may be used temporarily during periods of expected shortages. Contingency capacity strategies should only be implemented after considering and implementing conventional capacity strategies.</li>
<li><strong>Crisis capacity</strong> strategies are not commensurate with U.S. standards of care but may need to be considered during periods of known shortages. Crisis capacity strategies should only be implemented after considering and implementing conventional and contingency capacity strategies.</li>

Abstract
Background The global Ebola virus disease (EVD) epidemic of 2014/2015 prompted faculty at Emory University to develop an educational program for nursing students to increase EVD knowledge and confidence and decrease concerns about exposure risk. Purpose The purpose of this article is to describe the development, implementation, and evaluation of the EVD Just-in-Time Teaching (JiTT) educational program. Methods Informational sessions, online course links, and a targeted, self-directed slide presentation were developed and implemented for the EVD educational program. Three student surveys administered at different time points were used to evaluate the program and change in students' EVD knowledge, confidence in knowledge, and risk concern. Discussion Implementation of a JiTT educational program effectively achieved our goals to increase EVD knowledge, decrease fear, and enhance student confidence in the ability to discuss EVD risk. These achievements were sustained over time. Conclusion JiTT methodology is an effective strategy for schools of nursing to respond quickly and comprehensively during an unanticipated infectious disease outbreak.
